We are excited to welcome new patients to Rose Rock Family Dentistry! To help make your experience smooth and enjoyable, we’ve outlined a few key pieces of information below. Our office utilizes digital forms which are sent out prior to your appointment through text. If you are unable to complete them electronically we kindly ask that you arrive 10-15 mins early to complete them in office. Forms are also available using the link below. School and work notes are available to prevent unexcused absences.
Preparing for Your Dental Appointment
For patients requiring pre-appointment medication, please inform us in advance. We also ask that you bring any relevant dental records and recent x-rays, which can be forwarded from your previous dentist. Emergency cases are accommodated as soon as possible based on urgency, so please call ahead to discuss the nature of your emergency.
Insurance and Financial Information
We proudly accept a wide range of insurance plans, including Ambetter, Delta Dental, Humana, MetLife, and many others. Our team will help maximize your benefits, though please be prepared to cover deductibles, co-payments, and other balances not paid by insurance. Scheduling Policies and Appointment Guidelines
