We will make every effort to accommodate new hygiene patients in a timely manner. Please keep in mind hygiene appointments are scheduled six months in advance. As you may know, afternoon appointments are very heavily scheduled to allow us to accommodate as many patients as possible. School and work notes can be provided to avoid an unexcused absence.
We ask that new patients arrive to your appointment 10 to 15 minutes early to allow completion of the new patient forms. For your convenience the forms are also located above. You are welcome to fill them out and bring them with you to the appointment.
If you have a medical condition that requires medication before the dental appointment please inform our staff. Prescriptions for un-established patients will need to be filled by primary care until established.
If possible please have your previous dentist forward copies of your dental records and recent x-rays.

Our goal is to have emergency patients seen as soon as possible depending on the urgency of the situation. Please call ahead before you come in, this allows us to better assess the problem and determine the appropriate time need to treat the condition. In certain cases multiple appointments might be needed to properly treat the condition.
Please remember to bring your insurance information to your appointment.
We realize that emergencies and other scheduling conflicts arise and are sometimes unavoidable. However, advance notice allows us to fulfill scheduling needs and keeps the clinic operating at its most efficient level. Due to our one-on-one treatments, missed appointments are a significant inconvenience to your dentist, the office, and other patients.
The policies are in place out of respect for all of us, including you. Cancellations with less than 24-hour notice are difficult to fill. By giving last minute notice or no notice at all, you prevent someone else from being able to schedule into that time slot.
The Late Arrival and No Show Cancellation Policy are each strictly enforced. All fees related to such policies are due at the time of absenteeism and will be charged to the patient
NOTE: You will never be charged for a cancellation if it is made more than 24 hours in advance of your scheduled appointment time.
Patients are asked to arrive at (or slightly before) their scheduled appointment time. A grace period of 10-15 minutes will be permitted for unforeseen delays a patient may encounter while traveling to the office for their scheduled appointment. If a patient arrives more than 15 minutes late for their appointment, the patient will be given the option of either being seen that day as a walk-in, if the schedule permits, or rescheduled for a later date. This process will ensure patients who do arrive on time are seen in a timely manner.
Please keep in mind that we do offer our staff a one hour lunch daily and the office may be locked during this time.
A “No Show” is a patient who fails to appear for a scheduled appointment without providing a cancellation notice. Further, a rescheduled appointment that is less than the 24-hour cancellation notice is still considered a cancellation and is treated as such. There is a $25-$75 charge for no show visits. This charge cannot be billed to insurance and must be paid on or before the next scheduled appointment.
To assist the patient in keeping appointments, we will send text notification 1 week, 2 days, and 24 hours in advance. All reminders are documented. Following the reminder call/text (or appointment confirmation), the patient is responsible for canceling or rescheduling the appointment no less than 24 hours before the scheduled appointment.
If the patient’s phone is “out of service” or not receiving calls, the patient is still responsible for communicating with the office. If we are unable to reach you and have not heard from you, your appointment time may not be reserved.
